I updated to 10.5.4 yesterday, and as I expected, it wiped out Refit and booted straight into OS X.
I reinstalled Refit, but as I didn't expect, it still boots straight into OS X.
Possible change in 10.5.4 that defeats Refit?
System: 15 inch MacBook Pro 2.16 GHz (older than Santa Rosa, but don't know what chipset).

I'm using 10.5.4 on a 17in Macbook Pro 2.4 Ghz, and see exactly the same problem. It seems refit doesn't get run; the machine always boots straight into OS X. Reinstalling has no effect.
As yet I haven't tried a manual install, I have been installing from the installer package.

I'm having the same problem on my 8 core Mac Pro, yet oddly enough, my current generation iMac still works just fine, even though both machines have the same updates. I've tried removing everything rEFIt related and re-installing with no change. rEFIt still doesn't work, even though it used to on the Mac Pro. I even tried the full manual install with the same results. I use a wireless keyboard which doesn't initialize in time for me to hold down the "option" key and access my bootcamp menu, so I rely on rEFIt to switch between OSX and Windows (something I do very, very often) so it's starting to get rather annoying to have to keep a USB keyboard sitting next to me just so I can switch the OS... grrr. Any suggestions or help would be greatly appreciated.

Using rEFIt with boot camp on a Mac Mini in 10.5.4, and it works fine.
If I use the startup disk preference pane, however, the restart process skips rEFIt until the computer is powered down again.

try unplugging your computer, taking out the battery and holding down the power button for 30 seconds. It worked for me.

Strange! I wrote on 2008-07-08 that my Mac Pro was no longer offering the boot menu when MBP was (both on Leopard).
Yesterday I was running Parallels (using boot camp on a dedicated hard drive) but I had to kill it; nothing was moving. Since then Refit boot menu is back on MP?

Having the same problems with no rEFIt window on a mackbook with Leopard 10.5.4
Don't have a lot of experience, but I noticed this elsewhere on the site: http://refit.sourceforge.net/doc/c4s4_safesleep.html
"This behaviour is made possible by the “rEFItBlesser” Startup Item that is installed by default when you use the installer package to install rEFIt. When Mac OS X starts up, rEFItBlesser sets the boot options to boot directly into Mac OS X. When Mac OS X shuts down cleanly, rEFItBlesser changes the setting back to point at the rEFIt menu. (This mechanism also lets you install rEFIt without having to run enable.sh manually.)"
Now, I looked in my startup items and there is nothing, so I'm thinking that rEFItBlesser might not be getting installed correctly. Doing a spotlight search for 'rEFItBlesser' shows that the only instances of it are in the mounted install drive that I just decompressed from the .dmg
I'm not sure how to fix this, or even if this is the right direction... just wanted to share what I saw

Installation on mac mini core 2 duo, leopard 10.5.4:
- first install
- no refit after shutdown, startup
- had to unplug power cord after installation
- now refit works
- maybe this should be added to documentation

I found this on another site and it has worked every time: Turn off your Mac completely allowing it no power whatsoever, i.e. no power cord or battery. Let 'em simmer for a second and then boot up. Presto! rEFIT now works.
