I have had this bug twice now and I thought I should report it.
So in the last two consecutive upates after restarting I get (force) sent to the recovery mode.
MacOS is seemingly unable to boot and prompts me to reinstall MacOS.
The fix is to go Utilities -> Select StartupDisk -> Select the actuall HD
This will boot into the updates OS and everything is fine.
Its still a little unnerving when it happens.
Any idea what might be causing it?

OS updates (not just for macOS; for most OSes) frequently include updates to the OS's boot loader, and these updates typically reset the default boot loader to the OS's default, bypassing rEFInd, any other OS's boot loader, or any other user-specified boot loader. This is what I call a boot coup. There's no easy and general solution to the problem, but the rEFInd documentation page to which I've just linked provides some tips on avoiding or recovering from boot coups.
