Removing 'memtest' from showtools removes both icons. Adding it again = shows them both up.
The only remedy I've noticed is to remove \EFI\tools\memtest directory from ANY of EFI partitions. I think it's strange as I've thought the only one "refind.conf" was processed, so it seems like both EFI partitions have been scanned for some reason.

Most likely you've got two copies of memtest installed in two different locations. Try checking all of the FAT partitions, or other partitions that can be read by rEFInd via drivers, for memtest program files.

I've noticed: once two rEFInd instances installed on two boot drives (just in case), there are two memtest small icons appearing.
refind.conf includes:
scanfor manual
dont_scan_volumes "Windows7_SSD,Windows7_HDD,OpenMandriva,Fedora"
showtools about, memtest, reboot, shutdown
