The theme looks OK to me. Icons that are missing from a theme will show up either using the default icons or using an ugly "fallback" icon. Your screen shot shows neither of these symptoms.
It's unclear to me precisely what you think is missing, although your set of second-row tool icons is quite small, with just three items. This could happen because you've edited the showtools line in refind.conf to contain a limited set of tools and/or because tools are unavailable on your system. rEFInd won't display icons for tools like an EFI shell or memory test utility if no such tool is installed on your computer, for instance. Some tool icons are available only if there's support in the firmware, too; this is the case for the reboot-to-firmware tool icon, for instance.
If your first-row OS tags are incomplete, then rEFInd is not detecting a boot loader for whatever reason. If it's showing the wrong icon, then there are multiple options for correcting that problem; see this section of the rEFInd documentation for details.

Is the theme missing image files for the missing buttons? Compare the images between the theme and rEFInd.
