I'm using the refind 0.10.3. I'm using an iMac 21.5 2015 version with Mac OS X EL Capitan and Windows 10 64Bit with Bootcamp.
Mac OS X is not being detected by rEFInd. To boot into OS X I need to hold alt while booting and load it from there. I have tried fiddling with the sample OS X menuentry in refind.conf but I can't seem to get rEFInd to recognize the loader.
The Mac OS volume name is "Macintosh HD". I get this error when I tried the mmanual entry.
The EFI Folder has the Boot folder and the Microsoft folder will all the approiate files. The entry that refind find for Windows works and the manual entry as well. Refind just doesn't want to find the MacOS entry and the manual entry does not want to work.
I have 2013 version of this iMac with Windows 7 and refind finds both OS without a problem. I'm not sure if this is a new issue with Windows 10 EFI.
I'm NOT encrypted with FileVault so I don't know if I should still try boot from Recovery HD partion. If I have to try, I would like to try a manual entry so I could rename the menu entry.
Another thing that I noticed is that the refind-install script is installing the ext4_x64.efi driver. I thought this driver is for Linux partions. Could this be the reason is not seing the MacOS? Should I copy the hfs_x64.efi driver to the /EFI/refind/drivers_x64 folder? I'm going to try this to see if it helps.

OMG. I got it to work by copying the hfs driver. I can't believe I couln't ask this same question in my mind. Oh well, I guess It is brainstorming in text.
