    Hello,

I upgraded from macOS Sierra to High Sierra on a SSD installed in my Mac Pro 4,1 (flashed to 5,1) via PCI card. Everything installed without problems, also the transition to the new APFS file system happend and the Recovery HD was present. Also the firmware update which was required by the High Sierra installer prior to the system upgrade worked smoothly.
After reboot, rEFInd didn't show up anymore automatically which was to be expected. I decided to download and install the latest version available (rEFInd 0.11.0). As suggested, I choose the easiest way: disabling SIP via Recovery HD, reboot into macOS and installing rEFInd again with "refind-install". Before using refind-install, I adjusted the sample configuration file in order to reflect the same config I already had with rEFInd 0.10.8. The only change I made is to activate the following:
"scanfor internal, external, optical, cd, manual"
The default on Macs normally is "scanfor internal, hdbios, external, biosexternal, optical, cd, manual".
I already used that configuration with rEFInd 0.10.8 without problems, the reason to apply it was that rEFInd always showed more entries than I needed after I made a pure Windows 10 EFI install on a separate internal HD.

After reboot, rEFInd shows up as before upgrading to High Sierra.

BUT: It seems I have no way to access my Recovery HD any more. It neither shows up in rEFInd - as it did before upgrading to High Sierra - nor does Command-R work after restart to get to the Recovery HD.
Using Command-R only results in a black screen, monitor going into sleep mode, black screen, monitor sleep mode and so on...

As I wrote above, before updating rEFInd, I was able to access the Recovery HD this way.

Any help will be very much appreciated.

A known bug in rEFInd 0.11.0 keeps the macOS recovery (2nd-row) option hidden. This is fixed in my current development version available via git, or in binary form here:

    http://www.rodsbooks.com/refind-bin-0.11.0.1.zip

I'm currently investigating other High Sierra-related issues, and with any luck I'll make a 0.11.1 release before too long; but I have other (non-rEFInd) things on my plate, too, so I can't make any promises about timing.
