I had a dual boot setup using OSX/Win7, all was working fine. I installed refind, added another partition and installed Linux on the new partition. When the machine boots, refind launches, shows me all 3 OSs. I can boot OSX & Linux, but now, it cannot find Win7. I get "operating system not found" only for Win7. When I'm using OSX, I can see the Windows files on the BOOTCAMP partition so its still there, it just seems that refit can't find the WIN7 MBR.

You can probably restore Windows to bootability by creating a fresh
hybrid MBR using gdisk. The Windows boot partition must be included in
the hybrid MBR. It's conceivable you'll need to use Windows tools to
re-install the Windows boot loader from an emergency disk after you
create a fresh hybrid MBR, as well.

I had a similar situation. Dual boot with Windows 8.1 and Yosemite worked fine, then I divided the Windows partition and installed Ubuntu. At this point, only OS X was bootable. No Windows tools could fix the problem, but reinstalling rEFInd got all three partitions bootable again.

Chances are your Ubuntu installation has replaced your hybrid MBR with a
standard protective MBR. Consult the
gdisk
hybrid MBR information here:http://www.rodsbooks.com/gdisk/hybrid.html
