I installed rEFInd today and found that the provided Haiku icon is not an official Haiku logo, but a redrawn one. So I made an icon from the official artwork of the Haiku project.
I used Wonderbrush (Haiku image editor) since that's what Haiku artwork is designed with. I attached an SVG export but the shadow blur in it is not correct. The PNG rendering is how it should look like. Maybe you can use the SVG to apply the exact same shadow as in the other icons (I don't know what settings you used), or maybe you can use the PNG as-is, if you want.
I think my icon does fit the current style ("flat" look with shadow) unlike these previous submissions.
