When I reboot with BootCamp app in Windows, it seems to wipe rEFInd out. Any idea how to stop this, or do I just not use BootCamp app to reboot the MacBook?

When you use Boot Camp exe to select macOS, it means that rEFInd will not be the selected boot option anymore (nvram is modifed like how Startup Disk preferences panel does in macOS, or the bless command in macOS).
Hows is rEFInd installed in your Mac? I have it installed on a seperate HFS+ partition in /System/Library/CoreServices with a SystemVersion.plist file and there's a zero size mach_kernel at /
This setup allows rEFInd to show up in the Startup Disk preferences panel in macOS. Maybe it will show up in Boot Camp exe also?
If not, then rebless rEFInd in macOS. Next time you run Windows, do not use Boot Camp exe to restart. Just use the Windows Start menu to restart.
