Using a USB key, populated with ubuntu-16.10-desktop-amd64.iso from Mac USB Linux Loader v3.1, to install Ubunti onto an external USB drive, results in an extra boot entry in the rEFInd selector. This final entry shows the USB key icon on the Ubuntu icon and fails to boot properly due to insufficient video drivers. Removing all the extra kernels on the resulting Ubuntu volume doesn't eliminate this boot selector entry. Any idea where this one is coming from and how to eliminate it? The usable entry is the first one with a hard drive icon on the Ubuntu icon.
