The most common source of that problem is a bug in the EFI stub loader. Unfortunately, it's a very difficult bug to track down. To the best of my knowledge, it's most common on Lenovo laptops, although other brands have also been known to exhibit it. See this thread on the Arch Linux forum for an extensive discussion of the problem.
The only true solution at this point is to use ELILO, SYSLINUX, or GRUB rather than the EFI stub loader. A workaround that sometimes works is to switch from a version of rEFInd built with the Tianocore toolkit to one built with GNU-EFI, or vice-versa. I've got both builds available from the rEFInd downloads page.
