It might be worth the wait. I ran your test data and the document
through runbib and it works just fine on my box. There's only a few
minor issues like the citation key case. The citation keys must be all
uppercase for SGML documents. 0.9.3 and the current prereleases have a
new refdbd switch (-U) and config file variable (upper_citekey) to
automatically uppercase all new citation keys.

Please try again with the next prerelease, due in a day or two. There
might still be a platform issue so testing is important for me.
