[Refdb-users] missing scripts in 0.8.2.tar.gz

Hi all

the refdb-0.8.2.tar.gz archive regrettably lacked a few
important scripts. The maintenance release 0.8.2a adds these
scripts back so things should work ok now. I also edited
the readme.txt for the manual, as I no longer ship the
callout graphics. They come with every DocBook DSSSL
stylesheet release, so you have them anyway if you build
the manual yourself.
