#388 Ctrl-Alt-Enter does not work when remote end is missing

When remote end goes away (network issue, it died, etc) with rdesktop in full screen mode, it gets stuck - Crtl-Alt-Enter no longer works and the only way to get desktop access back is to log from another machine and kill rdesktop - very annoying. I think Ctrl-Alt-Enter not working should be considered a bug.
