#9 rapidxml_iterators.hpp improvements

1. Constructors take an optional "name", which lets you iterate through nodes/attributes of a given name only.
2. Ch defaults to char, just like the rest of the templates.
3. Fixed compile error on GCC due to superfluous use of typename.
