I had run into the same problem when trying to access the web based
management for the proxy server. It would just spit back the 403 forbidden
error and never prompt for a username/password. The fix was to make sure
that the DNS entries for the proxy server matched the hostname for the
proxy, and the browser was set to actually use the proxy. Once that was
done, the system would prompt as expected. I have not tried the username
based access for using the server in general, but those steps may fix what
you are seeing.
