#23 fix handling of XML namespace

The XML namespace is a built-in, so its attributes and attribute group definition are available to the binding generator, but it also needs to be available as a generated binding so other schema can reference those attributes. There's some sort of hack in there right now, but it's not clean.

Ended up making http://www.w3.org/XML/1998/namespace a built-in namespace like xmlschema is, with implementation in pyxb.binding.xml_. No longer generate archives or bindings for that namespace.
