The name of the file:

    pyPgSQL-2.4.win32-py2.1.exe

implies that the targeted Python version is 2.1 (py2.1), not Python 2.3. 
You can only use binary versions of extensions such as pyPgSQL with the 
version of Python for which they were compiled.  IOW, download the 
version for the Python you want to use, particularly:

    pyPgSQL-2.4.win32-py2.3.exe
