At long last such classics as ARB.multitexture now work on
Mac OS X! The trick was to get dynamic extension loading
to work. Here's a patch that does it.

Okay, I'm now checking that this patch doesn't break
anything on Win32 (which seems rather improbable given that
it's all inside ifdefs that will drop it out on all other
platforms :) ). Accepted and integrated. Only change was
to use C-style /* */ comments rather than C++ style //
comments. We have IIRC had problems with C++ style comments
mucking up pure-C compilers on certain platforms.
