pv2raw - v1.00 Release

This is the final (for now) code for decompressing SMaL RAW files.

What's New:
- added CFA post-processing routine
- identified new RAW file header fields (numDarkCols, fullTint)
- added support for old (Type-06 header) RAW files
- new API entrypoint which can get header, thumbnail or CFA data from RAW file
- cleaned so it's easier to port to different build environments
