I think the most important thing to work on for psdoom
would be toning down the monsters. All they should do
is walk around at a slow pace. It's hard to shoot them
when they're moving so fast, and hard to find the one
you want when they're all following you in a big group.

The 'follow you around' problem is mentioned in the TODO file:
--make pid monsters less aggressive towards (afraid of?) player
But perhaps also slowing their movement and/or having them
not attack anything would help as well.
Possible problem: I think monsters must be 'targeting'
something to move around, but I'll have to check that out.
Solution: slowing movement would probably be easier because
there is alredy a way to control monster speed. (-fast or
Nightmare skill)
