If there is problem with permissions for the directory
/var/www/projectbench/uploads/ (or what is your case)
and definite user can't create files there, operation
"attach file" fails without error message. It looks
then like file was really attached, but there is no
file of course. Possible result is data loss, because
submitter is quite sure that operation was done
successfuly.
