It is not possible to insert children between two siblings in a tree. That way NodeLinkTreeLayout is completely useless for dynamic graphs.

I have worked around this by using columns for PRED, SUCC, HEAD and TAIL entries and writing a custom VisualGraph. it would be useful to not have to subclass Visualization and hack it to provide a different VisualGraph implementation.
