Incorrect prefix when pretty printing hierarchical types

In some cases, the type name printed back to users does not exist due to an incorrect prefix. The attached example shows A.t is printed as A.B.t which is not a valid type name. More confusingly, the A.B.t may actually exist as a type that is distinct from A.t.
