#5 script won't create database

I cannot seem to get the install script to create a new database/tables.
I have tried several different versions of config.php but always get these results:

There seems to have been a slight problem with the database.

One item in the config I have not tried to change so far is the "full path" statement, it has been left at the default '.'.
Can someone point me to a sample config.php so I can see if there is any text I may have formatted incorrectly or give me any other suggestions?
