#14 Allow description text box when adding patch

When adding a patch to the plm, a text box should be on the
page to allow the user to provide a text description of
the patch. That way, when the information about this
patch is queried (possibly as a result of a search
function), a text description can be shown that
provides more detail about the patch. This will be
useful, since it is difficult to supply all necessary
information about a patch through its patch name.
