#4 Caching not working while output not ready

When the destination host for ssh tunnel not up, piper
restarts ssh. If we get input during the period ssh
tries to establish connection, its being written to ssh
(due to buffered i/o), and then ssh discovers that
connection can't be established and dies together with
the data sent to it.
