[pilot-db-list] CSV Conversion Problem

I could not convert any csv file with csv2pdb tool, (I dowloaded it
yesterday)

Even simple files create with notepad like :
John, 40
Mark, 30

csv2pdb return "at least one list view must be specified"

Did I miss something ?
What is the right command, I tried all options ??
