[pilot-db-list] note support in palm-db-tools

I am adding support for the note field to palm-db-tools. However, I need to 
know how the note's title and the note itself should be specified in the CSV 
file.

More than one field position in the CSV file? (And csv2pdb would combine the 
fields into one note field.)

Take the title from the first line of the field data like the Memo 
application?

Any other ideas?
