Initial Comment:
The item does show up in the content manager but 
when I click on any link like "add/edit my contact info" 
or any of the others i see:
"Fatal error: Call to undefined function: gettemplatedir() 
in /home/user/httpdocs/core/Template.php on line 158"

is this a layout issue?  I have installed some of the other 
extensions:
