#111 CreateToc plugin broken if headings contain '/'

If the CreateToc plugin is used on a page with headings
that contain forward slashes, error messgaes are
displayed. Turns out that the CreateToc.php plugin
doesn't quote forward slashes in heading text before
using them as patterns in regular expressions. This
means that the patterns are
being prematurely terminated. It seems the author had
got halfway round to solving the problem by
reimplimenting the preg_replace function in the
CreateToc class but just doesn't call it.

This is patch is writen for the latest version of
CreateToc.php taken from the CVS (1.26)

$ cd phpwiki
$ patch -Np0 CreateToc.patch
