Yes, I think we should leave it as an administrator configuration option;
at the moment the default is to dynamically determine the host name.
Admins can set the $ServerName to either the full URL or http: based on
their needs. I think this works for all cases, the choice is not hard to
make, and we don't have to risk introducing more bugs. Keep the details
out of the code and in the config file.
