I've got the MathML plugin working locally. You can see it in operation
on my wiki at:

I'll tidy it up a bit and see if I can produce a patch in the near future.
The only outstanding problem I know of it that the "preview" mode doesn't
display properly. That is due to a line in editpage.php which sets 
Content-Type to text/html. Unfortunately, when I change this is seems 
to break things, for reasons I don't understand.
