With the proviso that I run a (fairly recent) CVS PhpWiki: If I make a page and you edit it then that's a major edit (unless you tick the "minor edit" checkbox, as you mentioned). If I make a page and then edit that page again then that's a minor edit (unless I untick the "minor edit" checkbox"). If I make a page and don't edit the page for a long time (the exact time is configurable, but I think it defaults to something like 7 days) and then edit it again, that's a major edit. Those are the major cases, at any rate - I'm not sure if my list is exhaustive. As to the number of minor edits, this is configurable in index.php (look for MINOR_EDIT_TIMEOUT and the like), but the default behaviour is quite sensible. Using the SystemInfo plugin, mine is left as the default configuration, which is this: "Keep up to 8 major edits, but keep them no longer than 32 days. Keep up to 4 minor edits, but keep them no longer than 7 days. Keep the latest contributions of the last 8 authors up to 365 days. Additionally, try to keep the latest contributions of all authors in the last 7 days (even if there are more than 8 of them,) but in no case keep more than 20 unique author revisions." In other words, you should never get to have 256 minor edits on a page - the older edits will be discarded.
