rename install.php.php -> install.php

Afflicted and disciption are mixed up. Please reverse
these for both the add and edit pages.

When editing accounts, I should not have to re-enter
the user's name and password. these should be read from
the database and put into the form fields.

It would be nice to be able to add scopes.
