#9 schedafm.zip worksheet

On english language version, start page for not logged user contains on left side some information text. In that piece: "If you think anybody here matches into your family tree, please let me know or send me this worksheet.", word worksheet is linked to /phpmyfamily/schedafam.zip file. That file doesn't exist ins sources.
Could anybody tell me for what is that funcionality and what kind of worksheet file schedafm.zip should be ?

This is an artifact of a badly merged change from somebody - the reference to the worksheet should be removed

I would like to change the schedafam.zip link to point to an html form in a new subdirectory. How might I achieve this?

I think it should just be a matter of changing the link in the file(s) in the lang directory

I now see there is a built-in form so I will use that, however when I do I get an error saying
Message could not be sent.
Mailer Error: Language string failed to load: Authentication failed
Do you have any suggestions as to what could be wrong?
