checkboxes: place form tags in table.inc?

For my mysqlquery, I use ‚table.inc‘ + ‚sqlquery.in‘. In ‚table.inc‘ I
activated the checkboxes. In order to send the array–values of the chosen
checkboxes by a submit-button to another php3-script, I tried to place the
form-tags <form action...>...</form> into the table.inc &#8211; document, but I
didn&#8217;t find a way where to place it. Anybody knows a way how to do this?
