Further to below, I have got to the Configure User Page, entered info and
then get the following:-

User created successfully.

"Click to Continue" brings me back to a blank form again. 

There is no ./index/authenticate.php and no authenticate.php in the root
directory, as in the last version. I have set write permissions in
authentication_index.php in case, but this still doesn't work.
