#944 More flexible recent changes

At the bottom of an edit popup:

NOTE: if entered, this will appear in place of xyz in the recent changes block on the welcome page.

If anything is entered, put it in the note structure of the CHAN record. (5.5.1 spec p. 31)

In the changes block, if a CHAN record has a NOTE, use that for column two, otherwise do like now.

In the config, "when" should have three options:
- date
- date and time
- neither (omit the column but still put it in that order)

For "by who" allow the option of "real name" instead of user ID.
