#439 RE: Extended Relatives Report

First, thanks to tomvigl for his work on the extended
relatives report! I have made a few changes to the
format to get rid of some of the wrapping that occurs
on US letter format and included it here. Apologies in
advance: I didn't do any internationalization to this
file -- mainly because it would complicate the upgrade
process. This file should work without any
modifications to any other PHPGedview sources.

Now, for the RFE(s):
1. It would be great if the Descendants option could be
expanded to include spouses of descendants as well.
2. I'd like to add all notes associated with an
individual to the bottom of their entry in this report.
This becomes problematic because of the number of
different places notes can occur. If we include events
in the broad definition of notes, it gets even more
complicated. Any suggestions?
