In viewing Media for an individual, I have seen that the picture is visible, all information appears ok, and yet it has a red highlight around it. I didn't spot what that meant, but I think I figured it out indirectly by checking the GED. Entries like that shown below are the malformed ones (note the lowercase 'm', where this gedcom config requires an 'M'). I'm not certain how they became malformed, since I rarely edit the raw GEDCOM record data, but a quick edit solves it. I have corrected a few similar items in Note records.

You can get those "malformed" Media or Note references when you manually enter these numbers without paying attention to the case. As far as PhpGedView is concerned, you don't actually have to adhere to the case of those references when you enter them manually.
The red border means that the item has not yet been accepted into the database. You can avoid that problem by setting your own account options to automatically accept changes into the database. That option is there so that changes made by other, less knowledgeable, users don't appear in the database until an admin has verified their accuracy and adherence to site standards.
