#2380 [4369] problems when two media files have the same name

If you have a media file in a subdirectory that has the same name as a file in a parent directory, PGV gets confused over which file to lookup in the db.

For instance, if you have these two files:
/pgv/media/path1/file.jpg
/pgv/media/file.jpg
and you try to view this file using the media firewall:
/pgv/media/file.jpg
pgv may show you the wrong file.

The issue is that get_media_id_from_file() just returns the first match it finds, which may not always be correct.
