#1408 RA media error

I have created Sources for my census records and I attach the jpg image of the census page to the Source.
I then use the RA to enter the data for all the people and I use the Source reference there. (with no additional media)
When I complete the form associated FACTS are correctly attached to the individuls however one or more incorrect OBJs are also attached to the census image. These OBJS reference undefined media items and I sometimes get 4 or five of them.
I am at the current SVN but this seems to have happened a little while in the past.
