[Phpformgen-users] Error creating mysql table

I downloaded the latest 2.9c script. When running through the wizard on win2000 system and apache, I am getting the following error. I have given the correct info and changed the properies of files as recccomneded.

Could not create MySQL database table. Maybe a table of the same name already exists?

Can any body help?
