I downloaded PHP form generator from fantastico and created a call me back form; it appears to be fine but when I filled a test data, I did get this error
"Internal Server Error
The server encountered an internal error or misconfiguration and was unable to complete your request.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

Anyone got an idea what I would done wrongly? I am ready to provide as much information as possible to get this sorted. I have spent 5 days on this issue I really cant get my head round it.

there is a problem with a setting in your php.ini file. check with your hosting service to modify the php.ini file.
