MySQL form works for me, not for others

I've used phpFormGenerator to create a feedback form on my website which drops data into a MySQL database. 

I am able to successfully use the form from my machine.  However, when others use the form, their info is not posted to the database.

Any advice would be greatly appreciated.

hmm, that doesn't seem right. The program really doesn't know *who* is using the form :P
