If you can copy files, you can load and use this form.  There are occasional issues but for th remost part it is load and go.

Given your appearent lack of knowledge I would not attempt to test it as this would require setting up your own web server on your local PC.
IF you have a web site you must have load html code.  You would load your form the same way.  If you don't know how to do this then you are really going to have to learn a few things before you get started with forms.  Either that or get someone to help you out.

      Hello,
      Follow these steps:
1- make a new folder on your website server name it for example "myform".
2- Unzip the file "form.zip" which you got from "formgen.sourceforge.net" and copy all extracted files to "myform" folder on server.
3- change the permission of "myform" folder and also "files" folder inside it to permission "777" .
4- goto you browser and start running "install.php".
5- if you was select to send the data to email while you design your form, i think you will not get any questions while instillation.
If you was select send to database, you should enter your database info, such : host name, user name, database name, database password, and table name, do you know any thing about this?!!
i will may tell you about them later:)

      thank you

How do you change the permission to "777" in step 3 above?  How do you do that?  Thanks - CC

with your FTP client program like Filezella or any ftp client program, press right click on the file or folder you want to change its permission and you will find the appropriate command to this, it is deffer from one program to other.

Thanks, Essam - but, unfortunately, there is no such place for me to change permissions on my files using program WS_FTP running under Vista Home Premium.  I think I'm just going to have to re-do the entire form and try it again.  I've made a couple of short sample forms and uploaded them to the server, and they work fine.  Something went wrong with this form at the time it was zipped or modified (I'm the one that had trouble with the builder not accepting apostrophes in the field text).  It should be as easy as unzipping the files to a folder, uploading to the server, and it working.  This is how simple this has worked for me in the past.

      Thanks anyway.  CC
