I created the application - I downloaded the form, but I'm getting an error message saying that it was unable to unarchive - operation not permitted.

It's a zip file. I don't know what you are doing but you should unzip it like you would any other file and then copy the files to your web site.
I am having the same problem, it won't unzip basically, is this apple mac friendly?
