These problems are usually related to server php settings or file permissions.  Discuss the issue with your host first and if they think it should be working properly come back and add to this post again.

Then you have the same problem and need to follow the same advice given previously.
        https://sourceforge.net/forum/message.php?msg_id=4223936

Start with that and come back if you still need assistance.

      OH, I JUST SOLVED IT....

If you follow the path of where the error message is coming from (in my case process.php) you have to make sure the permissions for that SPECIFIC file is set to "755"....

      Thanks

If you navigate to the submit folder, I assume this is where the generator is located, you will see a warning page.  This warning indicates a standard generator file is not writable.  I believe this file is used to create a temporary copy of the form you are generating.  If this file is not writable then you may not be able to properly create a form.  This may be why your error is occuring.
        Try solving this error first.

Here is the text of the message:

        phpFormGenerator installation verification

You are seeing this page because you did not follow the steps mentioned in the README file that comes with this program. This page is for installation verification, and will ensure that you have performed all the steps required for this program to work properly. Please take a look at the following message and fix any errors.

        File and Directory permissions
The forms/form1.html file is not writeable.
Please give read+write permissions to all the files and directories mentioned above. Refresh this page after you have done so.

I'm getting that same "internal server" error.

My hosting company offers automatic installation of this form via fantastico in my control panel.

I've tried every suggestion posted above, and am still not having any luck.

Even tried installing it from scratch and not using fantistico automated installer and that's not working either.

Unfortunate, as I really like the ease of use as far as generating the forms, it would just be nice if it actually worked correctly!

Plenty of people are using this program with great success.  The errors can usually be traced to file or folder permissions.  Have you host review your installation and verify the file and folder permissions.  Also suggest that they review the log files to see when and where the error occurrs.
I have this program installed on my local PC and on multiple host accounts.  I have had some problems but have always been able to get it working.

This is the precise problem with the software. There needs to be some very clear documentation written that includes instructions for newbies. How can anyone using this software have any confidence that it is working properly (not dropping requests, and not leaving significant security holes) if they have these kinds of problems and can only get a solution that says go back and ask your service provider?

No server side software like php is completely independant of the server (host) system.  There are literally thousands of combinations of server setups that would have some effect on how the software installs and operates.  It is not practical to attempt to cover all possible solutions either in documentation or in software design.  If you don't appreciate that fact that this software is completely free and it comes with an active support forum, then you need to find your solution elsewhere.

I did find the solution elsewhere.

I deleted phpformgenerator, deleted the databases I corrupted using phpFormGenerator, purchased a forms software that offers more support, and started from scratch.  I expect my business to have a future - I would like to believe the form generation software I am using will have one as well.  That is not likely with phpFormGenerator since it has to drag someones collosal ego along with it.
