Here is the site I'm trying to add a form to. It's hosted on the JustHost webserver, and I'm using Website Builder to make the pages and everything. I created a comment form here on FormGenerator, and I downloaded the ZIP file okay, but now I'm stuck.
How to get the form to show up on my website? I tried copy/pasting the CSS, but all that got me was a giant Wall-O-Text.
Any and all help is greatly appreciated.

I did not figure it out, actually. I ended up using a completely different form generator that was fully customizable and much easier to use. Unfortunately, I've lost the link since then and now I can't remember the name of it. If you're using JustHost as a server, though, in the cPanel will be a list of things you can use to add on to your page. Otherwise, I'm really sorry I can't remember what they are.

Basic installation is as simple as copying all files and folders as they appear in the zip file to some location on your site. Point your browser to the form.html file to show that this was successful. If not then there is likely an issue with file or folder permissions after the copy.
Change folders to 777, and files to 664. Typical installation does not require PHP files to be executable, otherwise PHP files could be changed to 775 instead of 664.
Integrating your form into an existing page is much more complicated.

I did followed your instructions, everything went smoothly except in received the email of the filled form.. received nothing..
any idea on what went wrong..
btw, when i unzipped the form.zip i found many folders :
\form3\tmp\persistent\form249065
with these list of files:
Calendar<dir>
files<dir>
imgs<dir>
confirm.html
install.php
processor.php
form.html
style.css
monofont.tt
CaptchaSecurityImages.php
